Medical Records Request<\/p>\n\n\n\n

Responding to Your Medical Records Request<\/strong><\/p>\n\n\n\n

HIPAA regulations allow a healthcare entity up to 30 days<\/strong> to process copy requests for medical records. We generally complete requests prior to the allotted time permitted, but due to the possibility for a heavy volume of requests received, we cannot guarantee a specific date prior to the 30 days.<\/p>\n\n\n\n

You may request records by U.S. Mail, fax, e-mail, in person, or over the phone.<\/p>\n\n\n\n

Please complete the attached form: DBHDS AUTHORIZATION FOR USE\/DISCLOSURE OF PROTECTED HEALTH INFORMATION<\/strong> when requesting medical records.<\/p>\n\n\n\n
